Medication orders: In order for a nurse to administer medication there must be an administration of medication form completed by the physician, signed by the parent, submitted to the school nurse with the medication in its prescribed container.
Medication Forms Physical Education Excuses, Crutches and Elevator Passes: Students that require an excuse from physical education (gym) or an elevator pass must have a note from the physician/health care provider for crutches, orthopedic braces or any other medical appliance.
Athletes: Sports registration is required every sport season. Information pertaining to registration is available on the
athletic website.
Annual Screenings: Screenings are conducted for height, weight, blood pressure, vision, hearing and scoliosis according to a schedule set by the State of New Jersey.
