Vaccination Requirements & Immunization Information for 6th grade

The New Jersey Department of Health and Senior Services requires the following immunizations for students entering sixth grade. These requirements state the following:
Every child born on or after January 1, 1997, and entering grade 6, is required to receive one dose of a meningococcal-containing vaccine (such as Menactra). The student must be 11 years of age to receive the vaccine.
Every child born on or after January 1, 1997, is required to receive one dose of Tdap (Tetanus, diphtheria, acellular pertussis) if the last dose was given five or more years ago.
HPV vaccinationis not required by the state of New Jersey, but vaccine information is included here for your information.
Please review your child’s immunization status with their physician and schedule any needed appointments for immunization as soon as possible. Once your child has received these immunizations, please provide the school nurse with documentation so we can update our records accordingly. Since vaccines are a state mandated requirement, students who are not vaccinated, or have not provided documentation of such, can be excluded from school.
