• Athletic participation is an opportunity for your child to get involved in the activities offered by Monroe Township Schools. Students are eligible to participate in interscholastic sports beginning in the 7th grade (6th grade for winter wrestling only). Athletic Registration is to be completed, and Athletic Forms are to be submitted to the school nurse by the deadline scheduled by the athletic department. 

    Athletic Registration is completed through FamilyID (Arbiter Sports). Registration must be completed for all athletes, every season. Health history update forms are no longer required as it is now part of the online registration. Once you have completed your registration, it will be noted as "complete" in your FamilyID account. Your child is not approved to participate in athletics until you receive a separate email with an approval notification. This will be sent after all paperwork has been reviewed by the school physician. 

    Athletic Forms have been changed. You are no longer required to submit the physical exam forms. These forms should be completed and maintained in your child's file at the office. The Preparticipation Physical Evaluation Medial Eligibility Form must be completed by your personal healthcare provider and submitted to the school nurse. You may access all forms here. The forms are also available in Spanish here.  When picking up the forms from your provider, please check that the form is complete. The form must be signed by your provider in two places, and should have an office stamp. If there are items on the forms that are incomplete, this will result in a delay in processing. Physicals are valid for 365 days. If your child is not due to have a physical until after the deadline to submit it, please call your school nurse for assistance. 

    Once the forms are received by the school nurse, they are sent to the school physician for final approval. This part of the process may take 1-2 weeks after being submitted to school. Once the nurse receives the approval from the school physician, your FamilyID account will be updated to "Approved" and a notification will be sent. 

     

    If your child has asthma, or severe allergies, please complete an asthma action plan or an allergy action plan as appropriate. If your child does not have these medical conditions, there is no need to submit these forms. 

    PLEASE NOTE THAT ALL ATHLETES MUST HAVE A COMPLETED FAMILYID REGISTRATION AND AN APPROVED MEDICAL ELIGIBILITY FORM ON FILE TO BE ELIGIBLE TO TRY OUT FOR ATHLETIC TEAMS.
