Athletic participation is an opportunity for your child to get involved in the activities offered at Monroe Township. Students are eligible to participate in interscholastic sports starting in 7th grade. Athletic physicals are to submitted to the nurse by the deadline scheduled by the athletic department.
FamilyID registration must be completed for all athletes, every season. The health history update form is no longer required as it is now a part of this registration. Once your registration is finished, it will be noted as "complete" on the FamilyID account. If you have questions about the website, please call FamilyID directly at the number provided on the website.
Athletic Physical Forms are required for all participants. Physical forms are four pages. Parents/guardians complete the first two pages and the physician/medical provider completes the last two pages. When picking up medical forms from your provider's office, please check the forms to make sure that they are complete. All areas must be completed, signed, dated and stamped. If there are items on the physical that are incomplete this will result in a delay in processing. Physicals are valid for 365 days. If your child is not due to have a physical until after the deadline to submit forms- please call the nurse for assistance. Once the physical forms are received and reviewed by the school nurse, they are then sent to the school physician for final approval. This process can take approximately one week after they have been submitted at school. Once they are approved by the school physician, it will be noted as "Approved" on your FamilyID account. If you have any questions about the forms, please call the nurse.
If your child has asthma, or severe allergies, please complete an asthma action plan or an allergy action plan as appropriate. If your child does not have these medical conditions, there is no need to submit these forms.
PLEASE NOTE THAT ATHLETES MUST HAVE A COMPLETED FAMILY ID REGISTRATION AND AN APPROVED ATHLETIC PHYSICAL ON FILE TO BE ELIGIBLE TO TRY OUT FOR ATHLETIC TEAMS.
