Enroll/Waive Benefits
Enrolling in Medical/RX Benefits - You will receive a benefit welcome letter from the state mailed to your home address explaining how to create an account with Benefitsolver. Benefitsolver is a website to enroll yourself (and dependent(s)) along with uploading supporting documents, if applicable (marriage certificate, Page 1 of 1040( if married) & child(ren) birth certificates). Attached is a document titled, MyNJBenefitsHub- Benefitsolver, which will assist you with setting up an account and how to begin the enrollment process. Once you start the enrollment process the program will walk you through step by step.

Enrolling in Dental and/or Vision - To enroll is Dental and/or Vision complete the attached Den & VSP Forms. Aetna Dental Enrollment Form (Sections C, D, G, H, and J), and/or the VSP Enrollment/Change Form, and return to my attention at Central Office or by email a few weeks prior to your start date. To enroll a spouse, I will need a copy of your marriage certificate and to enroll child(ren) I will need copies of their birth certificates.
Opting Out (Waiving) Benefits - If you choose to opt-out of benefits you will need to do this online in Benefitsolver. Once completed online, I will need the attached opt-out form returned to me along with a copy of your marriage certificate (if applicable), a copy of the benefitsolver waiver confirmation, and a copy of your existing medical insurance card, front and back. Please complete this step prior to your start date if you wish to waive benefits.
Opt-Out Payout
Employees who waive their School Employee Health Benefits and Dental and Vision benefits for other coverage are not eligible for a waiver incentive if their other coverage is with the State Health Benefits Program (SHBP) or School Employee Health Benefit Program (SEHBP) . This is a state law and not a district rule. If your coverage is not with SHBP or SEHBP you may qualify for an opt-out payout. The district has determined for those eligible the opt-out payout will be $4000.00 for family and $2500.00 for single if you are eligible to opt-out for the entire benefit year. If you are eligible and your start date is 8/1 or after the opt-out payout is pro-rated.
