To protect employee privacy, VSP no longer uses an employee’s social security number as their member ID. All member ID numbers will be your Employee ID number followed by 5 zeros, to create a 9-digit number (i.e., Employee ID number 1234 will be a VSP ID number of 123400000).

You typically have six months from the date of service to submit for reimbursement. Please review your plan information for deadlines and details. There are two ways to submit a claim to VSP.- 1. Your Provider Submits a Claim
You'll pay your provider any applicable copays and taxes, plus the amount over your allotted coverage. Your provider will submit a CMS 1500 form to VSP. VSP will pay the provider directly for your services and eyewear. Not all providers will submit a claim to VSP, ask the provider before you receive services. - 2. You Submit a Claim for Reimbursement
You'll pay the provider in full for the services and eyewear received at the time of your appointment, including taxes. Then you'll submit your receipt with an itemized list of services and eyewear along with the VSP Member Reimbursement Form to VSP. VSP will reimburse you the allotted amount based on your coverage.
