• Health Benefits: Leave of Absence


     

    MATERNITY LEAVE

    Once the baby is born, an employee has 30 days to submit enrollment/change forms to add the baby to your health benefits.  The baby will be covered automatically for 30 days.  Below are the carrier enrollment/change forms which need to be completed to add the baby to your health insurance plan(s).  Return the completed forms, along with a copy of the baby’s birth certificate and social security card, to the Benefits Coordinator at the BOE within 30 days of birth.  If the 30 day limit is approaching, you may submit the enrollment/change forms without the other documentation, and submit those copies upon your receipt.

    Aetna Medical-RX , Aetna Dental PPO , and VSP

    The Benefits Department will monitor the employee's health benefit contribution deductions each pay period.  When and if unpaid time begins, the employee will receive a monthly invoice to collect the monthly health benefit deductions.  The employee will continue to receive a monthly invoice until such time that the employee returns to work.  All 10-month employees are exempt from payment during the months of July and August.

    MEDICAL LEAVE
