Please be specific with the symptoms of their illness. You may leave a message at any time. Notes regarding the absence should be sent in when your child returns to school. A note from the physician is requested if your child misses 5 or more days from school. If your child develops a medical problem, please notify the nurse. In order to assure that your child receives the best possible health services, it is extremely important that we have a complete updated health form.Any new student entering school must have a current physical. Students entering from another New Jersey school should have their physical forwarded as part of their health record. Students entering from other states or countries must provide a current physical exam (done within the last 365 days) within thirty (30) days of registering.
