This is the Health Benefit Costs Chart for the period of 01/01/25 - 12/31/25. You are eligible to enroll only in the NJ Educators Health Plan or the Garden State Health Plan if your hire date is on or after 7/1/20.  If your hire date is before 7/1/20 you can choose any plan. To determine your Dental and/or Vision per-pay costs, look at the annual cost, multiply that amount by the appropriate percentage from the Percentage of Premium Chart, and divide by 20 (10-month employee) or 24 (12-month employee).  Add the amounts to determine a per-pay deduction. 

 

 

This is the Percentage of Premium Chart for the MTEA, MTSAA, MTBOESSA, Teamsters, and Non-Affiliates.  Use this chart to determine your per-pay health benefits deduction for all Open Access Medical/Prescription Plans, the Dental Plan, and the VSP Vision Plan only. This chart is not to be used if you have NJEHP or GSHP medical benefits.

This is the NJ Educators Health Plan Contribution Chart.  Use this chart to determine your per-pay health benefits deduction for Aetna NJEHP Medical/Prescription coverage only. You may determine your Medical/Prescription per-pay costs by finding your salary bracket, go across to the plan tier you are electing, multiply your salary by the appropriate percentage, and divide by 20 (10-month employee) or 24 (12-month employee).

This is the Garden State Health Plan Contribution Chart.  Use this chart to determine your per-pay health benefits deduction for GSHP Medical/Prescription coverage only. You may determine your Medical/Prescription per-pay costs by finding your salary bracket, go across to the plan tier you are electing, multiply your salary by the appropriate percentage, and divide by 20 (10-month employee) or 24 (12-month employee).
